‍ ‍Important: When you click “Book Now” or any scheduling link on this Website, you will be redirected to and will complete your booking on Aesthetic Record, a third-party, HIPAA-compliant electronic medical record (EMR) and scheduling platform that we use to manage appointments, patient charting, informed consent, and payment processing.

‍ ‍•      Aesthetic Record is not owned or operated by Nara Luxe LLC. Once you leave our Website to book, your information is processed under Aesthetic Record's own privacy and security practices, in addition to our HIPAA obligations.

‍ ‍•      Payment for services is processed via AR Pay, built on Stripe's infrastructure, in accordance with PCI-DSS security standards. We do not store your full card number.

Nara Luxe LLC d/b/a Nara Luxe Wellness (“Nara Luxe,” “we,” “us,” or “our”) is required by law to maintain the privacy of your Protected Health Information (“PHI”), provide you with this Notice of our legal duties and privacy practices, and abide by the terms of this Notice as currently in effect.

Other than as described above, we will not use or disclose your PHI without your written authorization, including for marketing purposes or sale of PHI. You may revoke a prior authorization in writing at any time, except to the extent we have already relied on it.

Your Rights Regarding Your Health Information

•      Right to Access: You may request to inspect and receive a copy of your PHI.

•      Right to Amend: You may request an amendment to your PHI if you believe it is incorrect or incomplete.

•      Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.

•      Right to Request Restrictions: You may request restrictions on certain uses or disclosures of your PHI; we are not required to agree, except in certain circumstances involving disclosures to a health plan for services you paid for out-of-pocket in full.

•      Right to Request Confidential Communications: You may request that we communicate with you about your health information by alternative means or at alternative locations.

•      Right to a Paper Copy: You may request a paper copy of this Notice at any time, even if you agreed to receive it electronically.

•      Right to be Notified of a Breach: You have the right to be notified in the event of a breach of your unsecured PHI, consistent with applicable law, including Florida Information Protection Act (Fla. Stat. §501.171), which requires notification within 30 days of a determined breach.

Third-Party Platform Note

Your PHI, including intake forms, consents, and treatment records, is collected and stored through our scheduling and electronic medical record platform, Aesthetic Record, which maintains its own HIPAA-compliant security safeguards. We maintain a Business Associate Agreement with Aesthetic Record consistent with HIPAA requirements.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at [Grettel Santos/754-704-4936/concierge@naraluxewellness.com], or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.

Changes to This Notice

11. Governing Law

These Terms are governed by the laws of the State of Florida, without regard to its conflict-of-laws principles. Any disputes arising from these Terms shall be subject to the exclusive jurisdiction of the state and federal courts located in Broward County, Florida.

Business and Licensure

Nara Luxe LLC d/b/a Nara Luxe Wellness is a Florida limited liability company, wholly owned and operated by a Florida-licensed Registered Nurse. Services are provided in accordance with applicable Florida Nurse Practice Act scope-of-practice regulations and, where applicable, under the operating exemption available to wholly nurse-owned entities under Florida law. (Fla. Stat. §400.9905(4)(g)).

Clinical protocols and standing orders are overseen by our Clinical Director, [Yalenis Perez APRN 11024500], who provides supervision consistent with Florida law and applicable practice agreements, under established protocols pursuant to Fla. Stat. §464.0123. Copies of current standing orders are maintained on file and available for regulatory inspection.

Scope of Services

Nara Luxe Wellness provides mobile IV hydration therapy, vitamin/nutrient infusions, and GLP-1 weight-wellness program services. These services are elective and wellness-oriented. They are not a substitute for regular medical care, diagnosis, or treatment provided by your primary care physician or other treating specialists.

No Guarantee of Results

Individual results from any service vary based on each client's health status, medical history, and other factors. Nara Luxe Wellness makes no guarantee, warranty, or promise, express or implied, regarding specific outcomes, weight loss amounts, or wellness results from any service.

Telehealth and Good Faith Examination

Where services involve a telehealth consultation (including as part of our GLP-1 weight-wellness program), that consultation is conducted in accordance with Florida telehealth requirements, including an appropriate good faith examination sufficient to establish a proper provider-patient relationship prior to any prescription or treatment recommendation.

GLP-1 and Compounded Medication Disclosures

•      GLP-1 medications (such as semaglutide or tirzepatide) offered through our weight-wellness program may be dispensed as commercially manufactured products or as compounded formulations, depending on availability and clinical appropriateness.

•      Compounded medications are not FDA-approved in the same manner as commercially manufactured pharmaceutical products; they are prepared by licensed pharmacies in accordance with applicable state and federal compounding regulations.

•      As with any medication, GLP-1 therapy carries potential side effects and risks, which will be reviewed with you during clinical intake and informed consent prior to treatment.

•      You should disclose your complete medical history, including current medications and prior GLP-1 use, during intake to help ensure appropriate and safe care.

Controlled Substances

Nara Luxe Wellness does not dispense or administer DEA-scheduled controlled substances as part of its current service offerings.

Not Emergency Care

As stated in our Emergency Care Disclaimer, Nara Luxe Wellness is not an emergency medical service. Call 911 or go to the nearest emergency room for any medical emergency.

Informed Consent

Prior to any treatment, you will be asked to review and sign service-specific informed consent documents through our patient portal on Aesthetic Record. Those consent documents govern the specific risks, benefits, and alternatives to each treatment and should be read carefully.

Third-Party Platform Disclosure

Booking, charting, informed consent, and payment for services are processed through Aesthetic Record, a third-party HIPAA-compliant platform, and AR Pay (built on Stripe). See our Website Privacy Policy and HIPAA Notice of Privacy Practices for details on how your information is handled.

5. Cancellation Policy Summary

‍ ‍Notice Required

We require at least 24 hours notice to cancel or reschedule an appointment without charge. As a mobile service, last-minute cancellations affect our ability to serve other clients and cover the cost of travel, supplies, and staff time already committed to your appointment.

Late Cancellations and No-Shows

•      Cancellations or reschedule requests made with less than the required notice may be charged a fee of $ 50.

•      Missed appointments without any notice (“no-shows”) may be charged the full cost of the scheduled service or a no-show fee of $50, at our discretion.

•      Repeated late cancellations or no-shows may result in a prepayment requirement for future bookings.

How the Policy Is Enforced

The full, binding cancellation policy is presented to you and must be acknowledged at the time of booking through our scheduling platform, Aesthetic Record. Aesthetic Record's AR Pay system automatically applies cancellation fees to the payment method on file when the policy terms are triggered. This page is a summary for your convenience and does not replace the policy you agree to during online booking.

Exceptions

We understand emergencies happen. If you have a genuine emergency that prevents you from providing timely notice, please contact us directly — exceptions to the cancellation fee are considered on a case-by-case basis at our discretion.

6. Emergency Care Disclaimer

‍ ‍Nara Luxe Wellness provides mobile IV hydration therapy and aesthetic/wellness services by appointment. We are not an emergency medical service, urgent care provider, or substitute for emergency medical treatment.

If You Are Experiencing a Medical Emergency

Call 911 or go to the nearest emergency room immediately. Do not use this website, our online booking platform, text messaging, email, or social media to seek help for a medical emergency, as these channels are not monitored continuously and are not appropriate for urgent situations.

Examples of Situations Requiring Emergency Care

Including, but not limited to: difficulty breathing, chest pain, severe allergic reaction (anaphylaxis), loss of consciousness, severe or uncontrolled bleeding, signs of stroke, or any other condition you believe may be life-threatening.

•      Our clinical staff will screen you prior to treatment, but our services do not replace regular care from your primary care provider or specialist physicians.
